Network Providers

Network Providers: The “network providers”, also known as in-plan providers, listed in this directory have agreed to provide you with your health care coverage. You may go to any of our network providers listed in this directory; however, some services may require a referral.  If you have been going to one network provider, you are not required to continue going to that same provider.  In some cases, you may get covered services from non-network providers.
Advantage by Managed Health Services HMO SNP (Advantage by MHS) is a Medicare Advantage Special Needs Plan with a Medicare contract.  Other providers are available in our network.  
How Bills are Paid: If you receive a bill from a non-contracted provider (out of network provider) for services rendered, do not pay the bill.  Please forward the bill to our MHS Member Services Department at: 10700 W. Research Dr. #300, Milwaukee, WI 53226 or fax to (866) 646-6090.  MHS will review the bill and determine if you have any financial liability.  MHS will also notify you of any financial liability.  Emergency Care: Emergency care can always be obtained in or out of the services area from the nearest available provider.
  • When in the service area you must use plan providers for urgent care. 
  • When out of the service area urgent care may be obtained from the first available provider. 
 
In addition, when out of the service area you can obtain dialysis treatment from any qualified dialysis provider. Please note: if you obtain routine care from out-of-network providers neither Medicare nor Advantage by MHS will be responsible for the costs. MHS Member Services is available at: 1-888-713-6180, (TTY, 800-446-6136) 8am until 8pm, 7 days a week to request information in alternative formats or languages.
