This directory provides a list of the  Advantage by Managed Health Services HMO SNP (Advantage by MHS) network providers and pharmacies.  To get detailed information about your health care coverage, please see your Evidence of Coverage. You will have to choose one of our network providers that are listed in the directory to be your Primary Care Physician (PCP). 

The term “PCP” will be used throughout this directory.  Generally you must get your health care coverage from your PCP.If you need assistance selecting your PCP, please contact MHS Member Services at (888) 713-6180 (TTY, 800-446-6136), 8am until 8pm, 7-days of week to request information in alternative formats or languages.
